Grooming terms & conditions-
As the owner or authorized agent for this pet, I hereby agree to the following:
I understand that my pet needs to be vaccinated to be groomed. The required vaccines are yearly Rabies, Distemper Parvo & Bordetella.
The person who brings the pet into the grooming department shall be the authorizing agent for all grooming instructions for the pet. This person shall be the witness to the particular cut, any matting, and the current length of hair on the pet.
Coat condition: I understand that this establishment puts my pet's comfort above all else. If my pet is matted, all matts will be removed. If brushing out is POSSIBLE, then a minimum charge of $10 will be added to the base grooming fee. I also understand that clipper burn or cuts can occur during the grooming process. I understand that all attempts will be made to prevent this, however in some cases, it does happen.. If any pet comes in with fleas/ticks, they will automatically be given a Flea Be Gone package (Flea shampoo & dip) at the owner's expense.
Emergencies: In the event of an emergency, I authorize this establishment to immediately seek professional veterinarian attention for my pet (at my expense). I understand that all attempts will be made to contact me in the event of an emergency. I understand that grooming can be stressful to some pets and I will inform the groomer if my pet has any heart conditions or any stress related issues prior to grooming. I also understand that if my pet has any stress relieving medication, we are authorized to administer it. A Place For Paws will not be responsible for any reaction the dogs may have to any medication.
All pets must be picked up within 3 hours of completion call or a daycare fee will apply.
If the pet is NOT picked up by closing time, a boarding charge will be issued.
If the grooming is deemed unsatisfactory, the person picking up the pet will be responsible for notifying the staff BEFORE the pet leaves A Place For Paws. The repair (if possible) must be done within 2 working days of the initial grooming. No money will be refunded if the repair is done. Money will ONLY be refunded (in part or in whole) if the error is on the part of the groomer and is NOT fixable.

Boarding terms & conditions -
I certify I am the owner of this pet, and I grant permission to A Place For Paws to obtain on my behalf and in my pets best interest the veterinary care necessary to treat illness or injury. I agree to pay all veterinary and other necessary services incurred by and for my pet during its stay in this facility. This boarding facility agrees to exercise all due and reasonable care to prevent injury or illness to my pet. However, in the event of illness or injury, the owner and employees of this boarding facility should not be held liable for such injury or illness. I agree to pay all charges the day I pick up my pet an understand that my pet may no leave the premises until the charges are paid in full. I understand that any animal left for five (5) days beyond the estimated day of pick up will be considered abandoned.
